All of the sections of the survey were from validated psychological questionnaires.
I'll clarify the types for you; there was a Menstrual Pain Questionnaire, a general Pain Questionnaire, a Patient Satisfaction Form and General Health Questionnaire. These are designed to pick up symptoms of mood disorders, catarophising etc. The extremities in selection possibilities are obiously a part of that and help to contextualise responses, especially so within the Menstrual Distress Questionnaire which asks for 3 measurement periods.
