A dear friend of mine is just finishing up her Masters' degree, and she needs women to fill in a questionnaire on the fascinating topic of Menstrual Pain, Distress and Primary Care. So please be a darling, and help her out!

Many thanks for your comments regarding the questionnaire that Andrew kindly passed on to me.

All of the sections of the survey were from validated psychological questionnaires.

I'll clarify the types for you;
there was a Menstrual Pain Questionnaire, a general Pain Questionnaire, a Patient Satisfaction Form and General Health Questionnaire. These are designed to pick up symptoms of mood disorders, catarophising etc. The extremities in selection possibilities are obiously a part of that and help to contextualise responses, especially so within the Menstrual Distress Questionnaire which asks for 3 measurement periods.
