Did the survey, but if you're acquainted with whoever put it together, could you mention that they should have included parameters for chronic conditions and/or space for the survey-taker to mention such.

For example, one of the questions asks if the respondent suffers feelings of low self-worth and, in my case, the answer is "Yes", but with the caveat that I've got a chronic mood disorder - it's nothing to do with my menstrual cycle (well, not much!)

And some of the words used in the "choose the term to best describe X" section were REALLY loaded - like, obviously so. If a person chooses to describe their menstrual pain as "horrifying" rather than simply "uncomfortable" that says a lot about the survey taker (and the folks putting on the survey!). If the survey takers were trying to be subtle, they failed, and if they were trying to use neutral terms, they blew it there, too. ;)

(a thousand pardons for going off. I'm a horrible survey taker, for obvious reasons)

Many thanks for your comments regarding the questionnaire that Andrew kindly passed on to me.

All of the sections of the survey were from validated psychological questionnaires.

I'll clarify the types for you;
there was a Menstrual Pain Questionnaire, a general Pain Questionnaire, a Patient Satisfaction Form and General Health Questionnaire. These are designed to pick up symptoms of mood disorders, catarophising etc. The extremities in selection possibilities are obiously a part of that and help to contextualise responses, especially so within the Menstrual Distress Questionnaire which asks for 3 measurement periods.
